This risk assessment tool uses information from the ARIC Heart Failure Community Surveillance and is designed for black or white adults ≥55 years old with heart failure and preserved ejection fraction (EF ≥50%) admitted to hospital with acute decompensation. It predicts risk of dying within 28 days and one year from admission to hospital.
